Region 64 Playing Rules Summary
Incident/Accident Form - fill out if an accident occurs (open with the latest Adobe Reader). Scan or fill out electronically and email to our Safety Director at safety@ayso64.org

Walk Home Alone Form - have parents fill out at AYSO General Release forms at beginning of the season for any child walking or biking home on their own.
Participation Release Form - have parent fill out after a player has been in an accident before returning to active play.
Equipment provided to coaches

If you need equipment after the start of the season or are returning equipment, please contact our Equipment Manager.

Coaches are responsible for bringing in a properly filled out Referee Game Card to every game

Properly filled out means that the team and player info is filled in -- only the referee should fill in the game card boxes, not the coach.

Use the free Coach's Game Card program (Mac & Win). The Coach's Game Card program does MUCH more than just create a Referee Game Card. It allows you to track how much playing time players get overall and in different positions so you can ensure Everyone Plays in a very fair manner. It prints out field maps and helps you be a Positive Coach by taking good notes during games.
